MEDICATION THERAPY MANAGEMENT (MTM)
Drug therapy problems occur everyday and affect the quality of life of many patients suffering from chronic conditions. By performing comprehensive medication reviews, our pharmacists are able to assess each patient’s medication for appropriateness, effectiveness and safety.Â
Patients having as many as five comorbidities benefit the most since medication-related problems are often found due to patients receiving care from multiple prescribers and/or pharmacies. Our MTM program includes medication counseling and patient education, which is pivotal to ensure adherence to medications. The program also comprises the quarterly follow- up evaluation of medications and an individualized care plan that achieves the intended goals of therapy.
DIABETES SELF MANAGEMENT EDUCATION (DSME)
Diabetes is a chronic condition that affects millions of Americans and requires a tremendous amount of education and behavioral changes. At WeLLatino we make sure our patients spend a sufficient amount of time with our providers to understand the disease state and the steps required to manage their diabetes.

Our Spanish speaking patients have the benefit of talking one on one their concerns and questions with our providers without the use of interpreters. Can you imagine trying to instill a behavioral change on a patient by using an interpreter? Yes! and we understand the struggle and this is why we ensure that our diabetes educators and providers are culturally competent and sensitive to the patient needs.
